There is a reasonably good chance that ssa3d30.ocx was already on your system when SQL Assistant was installed, but if not then SQL Assistant will install it.

A couple of people have suggested what might be causing it to get deleted (virus, or policy enforcement code).

Another (small) possibility is that something went wrong during the install and Windows Installer is somehow doing this.

If you can not find another explaination it might be useful to uninstall and re-install the product.
