Since the table is skewed I am guessing your insert is probably taking quite a while also. How many records on average are you inserting and deleting? You may want to consider loading the records to a work table and then use BTEQ to insert and delete. That is if your volumes aren't too high due to the Transient Journal overhead that you will encounter.

By the way a secondary index won't do anything but slow down the Multiload because then you have the index maintenance to think about. And that is only if it is a non-unique secondary index since you cannot Multiload a table with a unique secondary index.
