Originally it was isolated to just SQL Server data sources but recently it has begun to happen in some Informix data sources.

Currently we don't have any flat file (raw) data sources but that will change in the next month as we start to include Cobol unload from Bull- GCOS miniframe.

I agree with you it's probably time to open up an incident report.
