An access violation means that the DLLs are not playing properly in their use of memory, so you need to look closely at your MDAC environment. When troubleshooting ODBC the first place I look is at the version of MDAC. The version you're on has a pre-req of v2.7, but I didn't note that you'd done that upgrade.

If the latest MDAC version doesn't help download the MDAC Component checker from Microsoft . This nice little tool (cc_pkg.exe) will validate the health of your MDAC environment. Then two aspirin and a little egg-nog is in order.
