A null neither matches, nor fails to match, any specified value. Thus, -

WHERE X = :IN_x
>             AND (Y <> :IN_y OR
>                      Z <> :IN_Z)

does not yield a condition of Z <> :IN_Z since it might match. It is unknown. So, it won't be updated.
